My OA was about designing a system to meet a series of requirements, and the biggest factor was time pressure.

It wasn’t LeetCode-style, and I could tell from the prompt that the goal was to shape an end-to-end design under constraints.

I didn’t manage to fully complete the last part, and that lack of completion seemed to be decisive. I walked away feeling like I had the right direction earlier but ran out of time to fully finish the final requirement.

I didn’t receive an offer, and the experience basically taught me that the clock was part of the test.
