Problem

Describe a time when you failed.

Be specific about what you were responsible for, what went wrong, and how you responded once you recognized the failure. Explain the impact, the actions you took to repair or contain it, and what you changed afterward. We are probing for ownership, judgment under pressure, self-awareness, and whether you turn mistakes into durable improvements. A strong answer uses one concrete example, distinguishes your role from factors outside your control, quantifies the result where possible, and includes what you would do differently now.
