How would you design a RAG pipeline to minimize hallucinations while maintaining high throughput?

Discuss the full pipeline from document ingestion through retrieval, reranking, generation, and response validation. Explain how you would evaluate retrieval quality, groundedness, refusal behavior, latency, and cost. Address prompt injection in retrieved content and describe the tradeoffs behind your model, indexing, caching, batching, and context-window choices.
