Output

One row per month, facility, and service line, including unmatched lookup values as Unknown.
Columns: month, facility_name, service_line_name, encounter_count, completed_count, and completion_rate_pct.
Include only encounters from January 1 through March 31, 2025, and order by month, facility, then service line.
