Output

One row per active facility, with facility_name, region, completed_responses, top_box_responses, average_rating, and top_box_percentage.
Include only completed responses submitted from 2025-01-01 through 2025-03-31.
Sort by top_box_percentage descending, completed_responses descending, then facility_name ascending.
